Physical Nite 2024

Physical Nite Tuesday, June 5, 2024, 5 PM

For: Any middle or High School athlete requiring a Pre-participation Sports Physical Exam 2024-25 may participate.

Cost: $20 - cash or checks

Where: Broadway High School - main entrance 

Bring to Event: A  VHSL Physical Form (click here for form) - Complete all athlete information including the header of p. 3 (the Medical Provider will complete the remainder of p. 3).

Activity Registration for 2024-25 is now open. Please do so at this link: BHS Activity Registration 2024-25

Completing this form now will Coaches and Athletic Dept staff to know the names of students who plan to participate in each activity. Computers will be available at Physical Nite for registration, however, completing the form ahead of time will speed the process on June 5!

Additionally, Please, complete this Two Minute Survey (if you plan to participate). It will greatly help in event preparation!!

Expiration of Physical

(It is very important that returning athletes understand this)

All Physicals and Registrations completed before May 1, 2024, will EXPIRE on June 30, 2024. That means athletes who have a physical that was completed on or before April 30, 2024, will no longer be permitted to participate after June 30, 2024, until they have a valid physical completed and uploaded to their 203-24 Activity Registration.  

 Athletes who report to BHS workouts/tryouts without the 2024-25 Activity Registration completed with a current physical form uploaded, will not be allowed to participate! Staff is available in both the BHS and JF Hillyard Middle School office to upload physical forms for families. Please DO NOT give completed forms to coaches as coaches should not be handling physical forms!
